Description

Provides case management and housing assistance services to individuals with mental health disturbances. Helps transitional age youth between the ages of 18 and 25 years. Services include the following:

  • Mental health support
  • Intensive case management
  • Housing and employment support services
  • Independent living skills

Eligibility

Youths aged 18 to 25 years of age experiencing mental health disturbances
