Program: CALAIM COMMUNITY SUPPORTS - ACCENTCARE OF CALIFORNIA

Description
Provides the following CalAIM Community Support Services to Partnership recipients. Services provided include:
- Personal Care/Homemaker Services: Provides in-home support, such as bathing or feeding, meal preparation, grocery shopping, and accompaniment to medical appointments to members who require assistance with Activities of Daily Living or Instrumental Activities of Daily Living.
- Respite Care: Provides short-term relief for member caregivers. Members may receive caregiver services in their home or in an approved facility on an hourly, daily, or nightly basis as needed.
Eligibility
Must be enrolled in Medi-Cal through Partnership and Kaiser. Must have already applied for IHSS.

Proof of managed care plan in Sonoma County (either Kaiser or Partnership Health Plan).
