Program: CALAIM ENHANCED CARE MANAGEMENT - SONOMA THERAPY NETWORK

Provides Enhanced Care Management, which is comprehensive community based care management from a single lead care manager who coordinates all health and health-related care, including physical, mental, dental care, and social services.

Serves the following Adult Populations of Focus:

• At Risk for Avoidable Hospitalization or ED use,
• Birth Equity, Experiencing Homelessness,
• Living in the Community and at Risk for LTC Institutionalization,
• Serious Mental Health and or SUD Needs,
• Transitioning From Incarceration

Serves the following Youth Populations of Focus:

• At Risk for Avoidable Hospitalization or ED use,
• Birth Equity,
• Experiencing Homelessness-Homeless Families Or Unaccompanied Children and Youth,
• Involved in Child Welfare,
• Serious Mental Health and or SUD Needs (includes children with high ACEs scores)
